In an effort to provide our patients with a more efficient check in option, our office forms are now available online. Please note that all forms, whether filled out on the computer or by hand, must be printed out and brought in to the office at the time of your appointment.

If you are a new patient to our office, or have not been in for a year or longer:

Please show up for your appointment 15 minutes before your scheduled time.  This will allow us time to enter your information in the computer and take x-rays if needed.  Please download and complete the following form and bring them with you to your appointment or fax them back to our office at (805) 962-2154.
